Library-by-Mail Certification

Library-by-Mail at SJPL is a service that provides books and other library materials to San José Public Library members of all ages living in the City of San José. Library-by-Mail is only for individuals who are unable to visit the library due to a temporary or permanent physical disability.

Please complete the following certification form for the Library-by-Mail applicant to be enrolled in this service.
