- Phone: You may call 1(408) 808-2382 on Mondays from 10-11 am to make an appointment for the same day (Exceptions are legal Monday holidays including February 18 and April 1). There are only 10 phone reservation openings (6:00, 6:20, etc.) per week and they fill up quickly. Once all of the appointments fill up, staff cannot add additional times. Note: A reservation made by phone guarantees a consultation only if you are present at the exact appointment time. If it is for 6:40pm, you must be present then.
